Reservation Forms

In order for us to get your upcoming event on our calendars, we will need the form that is applicable for your event along with any deposits or fees associated.  Please note that park shelter reservations are only taken for the following:  Kid's Kingdom, Lions, Droste, South Riverside, South Riverside Pavilion, and Exchange Building. 

The Kohlmann Park Amphitheatre is the only area in Kohlmann Park that requires a reservation form.  Please utilize the 'Special Use Request' form to reserve the amphitheatre.  Shelters within this park are on a first come first serve basis.
