Sidewalk / Driveway / Curb & Gutter Replacement Permit Application

Completed permit applications along with the permit fee can be turned in to the Public Services Center or Inspection Services at City Hall.

Inspection of subgrade and formwork before pouring is REQUIRED to check for compliance.  
24-hour notice preferred.

Response time for shorter notice will vary based on Inspector availability.

PCC Construction Basic Specifications

These bulleted specifications are to serve only as a brief guideline to PCC work in the City ROW.  Contractors should reference the City of Waverly "PCC Construction Details" handout, SUDAS Standard Specifications and the City of Waverly's Municipal Design Standards for more detailed specifications/requirements.

  • Permit holder is responsible for notifying Iowa One Call prior to any excavation work and shall be responsible for any damaged utilities within the project area.
  • ADA compliant sidewalk closures shall be used for any public sidewalk that is closed during work.
  • All PCC work in the Right-of-Way should be Class C4 Mix.
  • Sand IS NOT allowed as a subbase for any PCC work in the ROW.  Aggregate subbase is to be compacted.  If ground is dry and adequately compacted, aggregate is not required, but preferred.
  • Sidewalk outside of driving areas shall be 4” thick minimum.
  • Approach and sidewalk thru driveway shall be 6” thick minimum.
  • All new or replaced sidewalk shall be ADA compliant with a target cross-slope toward the street of 1.5% (2% max). If connecting to a non-compliant area, sidewalk should gradually transition to compliant.
  • Rebar IS REQUIRED for curb & gutter tie-ins and must be epoxy coated with epoxy used in drilled holes.
  • Rebar is NOT REQUIRED for any other sidewalk or driveway approach work, but is allowed based on owner/contractor preference.
  • Expansion material IS REQUIRED between curb and approach, approach and sidewalk & between sidewalk and driveway above. Material shall be ½” foam or fiber board and sealed at completion.
  • All PCC work shall be broom finished.
  • All PCC work shall be saw cut as soon as concrete allows.  NO TOOLED JOINTS.
  • Restoration of the site adjacent to work shall be completed as soon as reasonable.  This may include backfilling, grading and/or street patching next to curb & gutter.
  • Any work performed without an inspection will not be accepted and may result in fines and/or required replacement with inspected work.
