A permit application must be submitted and approved before any water service line work begins. The permit fee is $25 and will be invoiced when the permit is issued. The contractor must submit (or have on file with the City) a certificate of insurance.
Note that an inspection will be required and drawings and/or photos of the completed work must be submitted to the City.

Materials Note: Service lines. 1” type “K” copper typical, with director prior approval 1” CTS poly (250 PSI) is permissible with tracer wire. Tracer wire must be installed on the outside of the curb box with accommodations made to be continued from the curb stop to the house. CTS poly must have stainless steel inserts, the use of compression fittings are required. No barbed fittings are allowed. For repairs to existing service lines, list the existing material type and material type to be used for repairs.
Materials Note: Gasket Joint SDR23.5 with crushed stone encasement (4" and 6" services) is required for all new construction. Schedule 40 PVC is required for sewer service entry within five feet of the basement/building. Any deviation requires prior approval from the Director of Public Works. For repairs to existing service lines, list the existing material type and material type to be used for repairs.
Note that this permit application must be completed and Iowa One Call (811) must be called prior to any work commencing.

City Coordinated Street Repairs
The City will coordinate the completion of a permanent pavement patch for the following fees. The City will invoice the applicant after the excavation is completed.
Pavements equal to or less than 3" in depth are charged at $4.24/sq. ft. Pavements equal to 4" in depth are charged at $5.75/sq. ft. Pavements equal to 5" in depth are charged at $8.00/sq. ft. Pavements equal to 6" in depth are charged at $8.50/sq. ft. Pavements equal to 7" in depth are charged at $9.00/sq. ft.
**Patching is generally done through a contracted project in the Spring and Fall.
